The deadline for completing the health questionnaire and health screening required by the Partnership Promise is June 30. To help you complete the health questionnaire requirement, the University of Tennessee has arranged for APS Healthcare to come to the Hermitage Room on the third floor of the University Center between 10:00 a.m. and 2:00 p.m. on Thursday, June 23.
Representatives from APS will provide the health questionnaire and can assist with questions. If you or your spouse have not completed your health questionnaire, we encourage you to take advantage of this opportunity. Be sure to bring your Caremark ID card with you to the Hermitage Room.
APS will not be administering health screenings on that day. If you have not done so already, you should make arrangements with your health care provider.
Note: the help session on June 23 is for UT employees who have not yet sent their health questionnaire to APS Healthcare. If you have already done so, then no further action is necessary.
